If you do not know, mark 1. If it does not apply, leave blank. Scores of 3–4 raise Evidence Confidence.
At the end you get dual scores: preparation vs demonstrated evidence — never an automatic “compliant” stamp.

Attention points
Seven statements are critical (weight 2): ownership, high-risk screening, sensitive data, change-driven reclassification, decision traceability, human oversight and AI-content transparency. A low score on them weighs more than the average suggests.
Dual scoring
Readiness Score measures how prepared your controls look. Evidence Confidence measures how much of that is backed by implemented, evidenced practice (scores 3–4). Separation avoids confusing optimism with demonstrated control.
